Patient labels may be sent by fax at (604) 909-4915 or via WhatsApp. Please indicate whether you would like me to see this patient for 1 prenatal home visit, 1 postpartum home visit or 6 weeks of postpartum primary care at home.

At this time, I am only able to accept patients residing in Ladner/Tsawwassen.
